What Could Cause Persistent Hoarseness For 35 Years Without Sore Throat Or Infection?

Hi, I have had quite bad hoarseness in my voice for around 35 years, sometimes its so bad I can hardly get a word out, no sore throat or infection, just redness around throat area, why would this be. I was treated for post nasal drip years ago which didnt help.
