What Could Cause Severe Testicular And Lower Abdominal Pain With Swelling?

Hi, I have recently been in hospital for testicular/lower abdomen pain and testicular swelling. They did an ultrasound and couldn’t see any ruptures or bad blood flow but also don’t know what the issue is. It is being treated as an infection but my blood showed no infection markers. My semen is also extremely clear, watery and thin. This is very unusual as it is usually white and thick. The pain is still very severe and My painkillers aren’t doing all that much. I also had testicular torsion around 9 years ago. Does anyone have any advice? Thank you.
