What Could Contact Dermatitis With Swelling Suggest?

i have been diagnosed with contact dermatitis, but i am worried. i have an ointment (mild steroid) to put on 2 times a day, but my rash is starting to swell into large red areas and the itch is almost unbearable. the only relief i get is putting ice packs on my abdomen. Should I be concerned and go to seek other help? How long should I wait to see the effects of the cream? I am in agony. :(
