What Could Ear Infection With Brown Discharge Suggest?

My friend's son has had an ear infection since December. He has had tubes in his ears, his tonsils and adenoids taken out...numerous visits with an ENT and not once has this "infection" cleared up. Now he has a reddish brown discharge with a foul smelling odor coming from his ear.
