What Could Night Sweaths, Chest Pain And Cough Indicate?

At night I have sweats, some pain in my chest, a cough that happens when I turn on my side. I had surgery in September 23, 2014, during recovery from surgery. I had blood clot form in my lung, on Coumadin. Can you help me to understand the what is going on in the evening.
