What Could Painful Breast With Protruding Nipples Suggest?

My girl friends says that her breast is paining terribly that she can even touch it slightly. Her nipples are protruding very much and very painful too. This pain is there for almost a month.She feels comfortable when she wears a bra. Can you please advice what could be the problem.
