What Could Painful Veins On Temple Region With Blocked Ears Suggest?

I have throbbing on both of my temples, I feel the vein throb. I've got a right nose nostril which has been blocked for quite a few years, and my ears I can control when they pop but it feels very unbalanced and I need to put this pain to a stop. Sinusitis or it's my ETD that needs to be seriously cleared. I'm 19 years old, Ive been to a few doctors about this in my home country and even saw a specialist. He wasn't that good and talked about surgery on my nose. If any advice or tips on what I can do would be greatful.
