What Could Persistent Rashes/cut On Hands Suggest?

Well, it started on my right hand of just a little red rash then started into cuts, the creases on the top of hand has cuts on it. It cracks when i move my hand. It has now moved to both hands. The cuts are getting bigger hand is tightening and my hands look like they are sweeling just a little. My mom has this problem, has had it for 20 years, and my moms dad had it for years and years. Could you tell me what it is?
