What Could The Sore Patch Near Back Be?

I have a sore patch on my scrotum near the back. It looks, as far as I can see which is difficult, a bit like spider veins. Sometimes the discomfort goes, but not for long. Do I need to see a doctor. I have IDDM, epilepsy, MI, hypothyroidism + meds for these if any could be relevant.
