What Could These Long White Coloured Plugs (for Lack Of

What could these long white coloured plugs (for lack of a better word) be that is all over my cheeks and chin? They are always in several clusters (more than 5-10) under a scab that is formed. They are next to impossible to extract; however, when you finally do, only then will the spot heal. The skin tends to get very dry and flaky around the edges. Any help would be greatly appreciated. What can make it better?
