What Do Shortness Of Breath, Dizziness And Pain In Chest Suggest?

My symptoms are extreme shortness of breath, dizziness, fatigue, chest pains that wake me from sleep and sometimes daily. I have an enlarged heart due to many years of treated hypertension, also i have had pericarditis twice. I am 52 years old, female with no other issues
