What Does Chronic Micro Vascular Ischemic Change Mean?

My brother has been told they think he has issues with his head...report stated "scattered areas of white matter and it was secondary to finding chronic micro vascular ischemic change. They thought he had had a stroke. Can you explain what this means? He's only 52 had a 5-bypass surgery last January
