What Does Constant Headache Indicate?

I have had constant headaches for 7 weeks every single day. I have pressure on my head like someone is squeezing my head. At night once I lay down I hear and feel noises like a rushing river and I feel the pulsating of my temple on the right side. During the day I can tolerate the pain with advil but the pressure never completely goes away. At night I have right side night tightening and shoulder and arm pain on the right. My headaches intensify every night. I have not went one day in the last seven weeks without this pain. Went to doctors office and was told I was healthy and she could see nothing wrong. I was told to listen to music at night for the tinnitus in my ears but that dont help. If I push on the little flap at the ear canal it calms some of the throbbing but not for long. After holding that in for 5 minutes it no longer have an effect. My vision has gotten very bad and I' m having hard times remembering things and lose my train of thought. I am scared. What could this be
