What Does Elevated BP And Light Headedness Suggest?

Recently, my blood pressure has been ranging from 160/100 to 150/55 to 140/50, to 135/60 - When it is at the high end of systolic pressure (150 +) I have symptoms of slightly light headed, foggy brain and some dizziness. With such a wide gap between systole and diastole, what might be the usual causes and what can I do about it?
