56 year old with no history of hypertension or diabetes, h/o chronic muscular pain,now 3 months of dry cough, chest xray showd Enlargement of cardiac shadow with prmoinent aortic knuckle, bilateral hilar congestion,accentuation of bronchovascular marking,and left lower lung zone pneumonic infiltration .. What could b reason regarding findings of heart?
