What Does The Following MRI Report Indicate?

I am looking for a top neurosurgeon in the Washington DC or Maryland area. Have seen a neurologist but the pain continues and the pain medicine is no longer helping. Have had a MRI which shows that L4.5 A broad base profusion is present at L4-5 as well as Facet arthropathy is also present with canal stenosis 0.5 cm A 4 cm synovial cyst or S1 the medial aspect of the left L4-5 facet. Moderate bilateral foraminal stenosis is also demonstrated at this level. Any suggestions for comfort?
