What Does This Sonography Report Mean?

Hii, we are recently informed by a MD in our area that my father has serious kidney problem ,we consulted a urology specialist and he recommended for sonography .the sonography report states that : right kidney is small in size .left kidney is enlarged . No evidence of any focal lesion is noted .no calculus in both kidneys.cortico medullary differentiation is poorly maintained with echogenic renal parenchyma . Right kidney measures 7.8 x 3.5cm ,left kidney measures 13.9 x5.6cm. Prostate gland is midly enlarged in size.it measures about 5.0 x 3.3 x 3.2 in size and corresponds to 28gm.small right kidney with compensatory hypertrophy of left kidney.bilateral medical renal disease. Mild prostomegaly . Doctor please explain it to me in a easier manner to me ....please and please sujest me the correct treatment to save his life.
