What Does This Test Result Of MRI Indicate On TIA?

I previously had a TIA (trans ischemic attack) in 2010, this affected my short term memory as well my speech. after extensive speech therapy and memorization excercises I had goten much better, since then I have suffered falls (3) and banged the back of my head in 2014, after the 3rd fall my speech and my memory was affected the same as it was in 2010. an MRI was done on my brain it showed bilateral periventricular white matter disease in keeping with a ischemic microangiopathy my question is Did I suffer another TIA? and what must I do to get better?
