What Is The Black Spot On The External Hemorrhoid?

I have a external hemorroid that is for about a month now and now it has a black spot on it and I don't know what to do? I have called my doctor and got some suppositories and a sitiz bath it seems like it has gotten worse? What is the black spot and should I be concerned?
