What Is The Recurrent Red Raised Rash On The Back Of Hands?

hi for the last 3 years i have had this red lumpy rash that appears on the back of my hands (mostly around my knuckles) somtimes on my knees and my toes it's very itchy it comes and goes every few months. i am seeing a dermatologist he has prescribed me plaquenil any ideas wot it could be
