What Is The Treatment For Severe Headache?

I have been having a killer headache since Jan. 20th..... I take topamax nightly and fioricet when needed, these meds have not been working at all, I've tried aspirin, excedrin migraine, and so today I went to urgent care and got a toradol shot, cambia 50mg powd pack, midrin, and ambien, and I am still wide awake with a splitting headache.....why is nothing helping it? Could it be a tumor?
