What Should I Do If I Have Low Ovarian Reserve, Took Progyluton Without Getting A Period, And Now Have Spotting After Starting A New Pack?

Hi. I have really low ovarian reserve. My age is 40 and im facing these problems from past 10 years. Im on snd off on progyluton and duphaston prescribed by my gynecologist. This time i took progyluton for 21 days but not get any periods. Now after waiting for 15 days i started new pack of progyluton but got spotting. Next day when i take again i get spotting. What should i do now?
