What Should I Do When My Foreskin Is Cracking, Tight, And Painful During Erections Despite Moisturizing?

Hi so I v been having trouble with my penis it has been cracking on the forskin and its tight when i pull it back and is pain full when i get erect as it splits Iv been trying to keep it moisturized to keep it from drying out but still doesnt seem work iv had trouble with this for Months.
