What Tests Should Be Done For Lapse Of Consciousness?

In 2009 I was coaching my son's football game and while giving halftime instructions I passed out. Since we were at a youth sporting event there was an EMT present, against my better wishes I was taken to the ER. I was thoroughly checked out and given a CT scan and all results came back negative for any abnormalities. I recently went to renew my driver's license and was told to call the Driver Safety Division. I called and was told that an ER reported that I had a lapse of consciousness and that I would need a doctor to fill out a "Driver Medical Evaluation" form. It was sent to me and it specifically asks the Doctor to evaluate the condition: Lapse of Consciousness. What tests if any should my doctor take?
