What Would You Recommend For Patient That Has Had 8

What would you recommend for patient that has had 8 spinal surgeries and have been using Demerol tablet for the past 5-7 years. WE now have a new insurance plan and they won't pay for Demerol. I can't take Morphine gives me Migrains and Oxicotton makes me itch. I don't want any thing that has additive qualities to it. Thank
