When Should We Seek Urgent Help For My Cousin S Severe Pain After Multiple Amputations From Meth Use?

My cousin has his toes amputated from meth. The er said for him to stop but he s been back another time & more plus another half foot amputated not he s in constant pain & won t go. When should we be real worried about it. We re already but I stay in the room with him & see him in more pain than anybody else & I just don t want him to die. Please lmk asap. tysm.
