Why Do I Frequently Forget Words, Use The Wrong Ones, Or Struggle To Form Sentences Even Though My Thoughts Are Clear?

hi doctor, i have never encounter any problem related to brain injury stroke etc to best of my knowledge. I face problem while speaking : sometimes I forget the words to explain myself, sometimes i start using wrong words although the picture is very clear in my mind but still i keep on speaking the wrong word for that picture, sometimes also i forget what I was saying if I dont find the correct word and sometimes I start speaking with lines correctly but while ending my lines I got confused to phrase my lines tenses etc. I dont think this is my major problem but yes it is my problem.
