Why Does A Male Of 65 Years Lose 5" Off

Why does a male of 65 years lose 5" off his height with no apparent reason? He use to be 5'9" and now is 5'4". He has been an alcoholic, has high blood pressure and high cholesetrol. I told him that there has to be a reason. Of course, he hardly ever goes to the doctor.
