Workers Comp. Ok's My Cymbalta One Month Then Doesn't Ok

Workers Comp. ok's my Cymbalta one month then doesn't ok it again for 2 or 3 weeks. I'm up and down month after month. Along with no sleep, being in a fog, sick to my stomach and suicidal now I have broken out in a rash all over that itches so bad I'm ready to lose it. The crying jags are crazy but this just puts the pain and depression in a much darker place in my head. I never get to stay on it 2 months in a row. I always have that 2-3 weeks it takes them to ok it after I run out. The rash is only getting worse and the dark thoughts are getting darker. Benydryl is doing nothing for the rash, please help.
