After 12 Years On 100 Mg/daily Of Elavil, Because Of

after 12 years on 100 mg/daily of Elavil, because of increased side effects dropped dose directly to 50 mg daily, after initial slight increase in pain ( one week or so) things seemed to settle down comfortably again, further reduction was then made to 371/2 mg daily with a goal for 25 mg daily, but sadly at this low dose I had an accident similar to the original one 12 years ago (epididymitis from wt lifting) this was 2 weeks ago, I have over this period increased daily dose to 75 mg with about 50% improvement only, but dizzy symptoms are so serious this time around that I find it difficult to continue.What to do????
