Hi, I Am 56, Have Been Noticing A Urine Smell

hi, I am 56, have been noticing a urine smell in my panties, so i wore a tampon today. When I took out the tampon it was soaked in urine and was hard to remove. Once I got it out, there was bright red blood on it. Im concerned something is wrong, what should I do?
