Hi Thank You. I Am A 59 Yr Old Diabetic

hi thank you. I am a 59 yr old diabetic with half dollar size diabetic ulcer on the plantar side of foot, under the main pad of tissue from big toe side of foot. I use mepilex to try to heel it but it has grown to a half dollr size, I am using provdone iodine sat dressings now. I don't believe its infected, but now the new tissue is surrounded by other layers of skin surrounding it . How do I get the layers of skin to even out smoothly?
