Hii I've Been Experiencing Itching In My Scrotal Area For

hii i've been experiencing itching in my scrotal area for a while now the area also has a dried reddish scaly appearance with folds. i saw my dermatologist he recommended use of some anti fungal creams with sertoconazol as a constituent. even after administering the cream for 3 months as recommended by my doctor the problem still persists to exist.
What should i do now?
