I Have A Thick Crust Of Hard Scabs Over Which

i have a thick crust of hard scabs over which grows skin and then another layer of scabs then skin. this has been going on for years. I have seen dermatologists but they don't seem to know. I have wrecked my shoulder picking at it . my friends are sick of watching me do this and so aqm I. I have also grown another ear lobe on my real one. it goes down my neck under my skin. I am desperate please help me
