My Friend Has Really Purple Balls And It's Happened To

my friend has really purple balls and it's happened to im before and lasted about two weeks he went to hospital and they couldnt tell him what was wrong but it went away on it's own but months later now it's back to purple again.any idea what it might be?
his cords inside werent twisted it's not loss of blood flow they look hickied?
