My Male Client Had A Traumatic Catheterization This Morning. There

my male client had a traumatic catheterization this morning. There was no urine return but the previous staff left the catheter in , hoping for urine. At 0900, staff alerted me that there was no urine but a small amount of blood in the tube. I advised them to remove the catheter since it was not in the right position and the client had a moderate amount of fresh blood from the urethra. The bleeding slowed, no active oozing and has been checked q2h. He did not void from 0600 till 1200 and there has been a small amount of fresh blood with each brief change. {developmentally delayed adult} Just now there is a small clot passed. Are we ok to continue to monitor or should he be seen?
