My Spouse Was Diagnosed With Stagae 1 Small Cell Lung

my spouse was diagnosed with Stagae 1 Small Cell lung Cancer August 2011. She went through radiation treatments in 2011-2012.. here cancer continues to grow. She was not a surgery candidate due to low testing scores for survivability. October 2014 the doctor told her she had about 18 months. She continues to smoke and drink like there is no tomorrow and has not been back to the doctor since. Recently she has developed a decent size lump on her neck, and her side (both right side) - Since she won't get this checked out, and I reasonable in thinking this is a sign her cancer has spread to the lymph nodes?
