Sir Good Morning , I Have Pain In The Neck

sir good morning , i have pain in the neck and shoulder.3 months back it started with sever pain in the neck -then the pain radiating to left shoulder and shoulder blade and continues in the upper arm and fore arm. taking rest and massaging with olive and other oils relieves pain. but when ever i sit on a chair to watch t v are computing OR RIDE A BIKE the pain starts in the shoulder and shoulder blade. my shoulder frame has tilted up on the left . xray report shows straightening of cervical lordosis and osteophytic lipping of vertebral bodies . several treatments didn't completetly give me relief. will u please give detailed advice about treatment , use of collar , etc.
