Started All Of A Sudden A Sharp Stabbing Pain In

started all of a sudden a sharp stabbing pain in my upper lobe of my back/shoulder area. Been going on about 6 hours now. almost like a spasm in my upper back/shoulder area. Now it is just stabbing and radiating around the area. any suggestions of what it may be?
