Question : 47 year old make with history of major depressive disorder (on Viibryd and Lamictal for a number of years). Also had hypertension (treated with low dose metoprolol, recently increased, and also recently added lisinopril). Wife is worried because she states that a few times when shes been awake in the middle of the night and early morning, she observes me twitching (mostly my legs) more than before. This does not wake her up, and states it is not terribly overt, but is noticeable.

                                    Brief Answer:
Twitching linked mostly to non-serious conditions.
Detailed Answer:
Hello and welcome to 'Ask a Doctor' service.
I have read your query and here is my advice.
Muscles twitching is a symptom that may be caused by several conditions, most of them not serious ones.
Benign fasciculations syndrome, electrolytes imbalance, adverse effects of drugs, etc. are common conditions that may be linked with such symptoms.
In rare cases, twitching may be linked to neurological disorders, but in such cases, other neurological symptoms are present as well.
I think that staying well hydrated, using magnesium supplements, should improve the twitching.
If the twitches get worse you should see your Doctor.
Hope you found the answer helpful.
Let me know if I can assist you further.

Periodic limb movement to consider also.
Detailed Answer:
Welcome back.
Since the twitching is happening during the sleep and you are unaware of it,periodic limbs movement should be considered also.
Periodic limbs movement during the sleep may be linked with antidepressants use as a possible adverse effect.
