Question: 55 yo f, history if PVC’s. Much worst when I used to over exercise, like high intensity training would make PVC’s (after workout) worst and make me tired, sluggish, headache for a couple of days. I stopped High intensity training. Never had Dx of long qt. Had ECG’s and saw cardiologist several years ago and diagnosis was PVC’s. PVC's where bothersome last fall and I had a holter monitor and the results where very few PVC’s and no more investigations. PVC's usually worst after heavy heavy exercise, too much sugar, coffee. I got pneumonia last January and doctor didn’t give me Z-pack because she was unsure if I had long QT syndrome and she gave me Doxycycline and did well. I now have H-pylori and (new doctor to me) prescribed Clarithromycin and metronidazole and I read that Clarithromycin is risky for long qt syndrome.
I have previously taken metronadozole, ciproflaxin and doxycycline before for other illnesses and did ok. Google is not always our friend but I can't help but look, when searching, I notice that many many drugs affect long qt. Do i really need to worry about this?

I don't have access to this (my doctor may have it but I don't) and when my (retired) doctor reported back on the PVC's to me, she then never mentioned concerns about long qt. Only when she prescribed me antibiotics in January did she choose not to prescribed azythromycin, she mentioned she wasn't sure If I had long qt (that was a new term to me at the time). I have no way to ask her this now, she's retired. A few different ecg over the years and no one mentioned long qt.
I do however have the apple watch ECG if that helps
Question: Could long qt come and go? be present only after the intense exercise and not show up on ecg? if so, would it be a risk factor?

See long qt syndrome is found in 1 in 7000 individuals and is more common in females. It is caused due to a genetic mutation inherited from parents and in most individuals it do not cause any major health issue.

If your previous ECG's were reported normal with no evidence of it then you need not worry for that. many medications have different side effects and its not a mandate that all individuals develop all those effects.

Now coming to your question, as its inherited so it doesnt come and go. If its not showing on ecg then chances are its not there at first place.

However, if you still wants a confirmation just get a 48 hours holter monitoring done again.
