Abdominal Pain, GERD, Gastric Disease. How Will The Pain Stop, Help?

Question: abdominal pain since 2005-2006, worsening last couple of weeks to daily pain. 2 EGDS found nothing. Dad died of Colon cancer. Seen 3 GI Specialist, cant no one seem to diagnose me and cure me. please help. They are saying either GERD or Gastritis Disease. Went to ER over the weekend gave meds. Woke up in pain, stomach killing me non stop. NO ONE can seem to help me, cure me no matter how many doctors i see. I cant take the pain anymore. I feel like im slowly dying. PLEASE HELP.

Hi and sorry to hear about your pain
In order to understand your problem better i would need some more information like-
1) The site and type of pain
2) Any radiation of pain
3) Any precipitating or relieving factor
4) The tempo of pain and it's behaviour over these years
5) Associated features like vomiting, bowel or bladder abnormalities
6) Effect of pain on appetite, weight, menstrual cycles and sleep
7) Investigation reports
Please get back to me with these so that I can suggest you better

Abdomen pain started back in 05-06. Seen 2 GI Specialist one at BMC and the other at BIDMC. Also had two EDGs (last one in Sept 2011) that just showed minor bacteria from taking headache medication. Last GI put me on Lansoprazole and Ranitidne, Prilosec and Zantac and NOTHING worked. I have taken Maalox, Tums, Pepto, XXXXXXX Seltzer, Baking powder with lime. I have tried almost EVERYTHING and NOTHING stops the pain. The pain has gone from 2 or 3 times a week to 7 days a week non stop abdomen pain. Sometimes the pain is so unbearable I cannot get out of bed that happens about once every 1 or 2 months. Now I am waking up EVERY morning gagging, naseous and feeling as if I have to throw up when I have yet to eat. I cannot eat full meals anymore and sometimes I dont eat at all because the pain is too much. I cant stand for too long because my stomach hurts and I have to bend over to eliminate the pain. The last maybe 2 or 3 weeks it has gotten WORSE. Went to ER on 3/30 and ER doc put me on Famtodine, Omeprazole and Ondansetron. By Sunday even after taking meds as described pain is back where it started. I have NOT had a ultrasound, CT or MRI. My dad died of Colon Cancer at the age of 55 being diagnosed at the age of 45. He was also a diabetic. His mom also passed as well as his father both with cancer. Mom Cervical cancer and his dad I am not sure what he passed with. ER thinks it may be GERD or Gastritis Disease. But no one can give me a definite diagnosis or treatment to STOP the pain. I have stopped going out, cooking and lost interest in almost everything I use to do. I had to drop out of my classes because my health became too much and my grades were being affected. Work is also becoming a problem because often times I am too weak and bent over to go into work because I cant stand long enough to get myself and my daughter ready. This has taken a toll on my life and I need HELP and a diagnosis and a CURE as soon as possible. I want to get check and eliminate or be diagnosed as soon as possible. Possible diagnosis can be gluten sensitive, IBS, H. Pylori, Pernicious, GERD, Bile reflux, ulcers, Crohn's Disease, Gallstones, Gastritis Disease, Kidney stones, and Appendicitis. I want to be checked for each and EVERY abdomen diagnosis there is out there. I have been to now 3 different hospitals and now 3 different doctors. The pain is on my lower right abdomen. Sometimes it can be sharp pains or sometimes just consistent pain, If I sneeze or cough it hurts. I have lost my appetite but make myself eat because I know if I dont it may get worse.

Hi and thanks for the folowup information
Your history is not typical of GERD or gastritis as -
1) Both never cause pain below the navel
2) The symptoms are very dramatic for GERD/ Gastritis
Of the differentials that you have suggetsed I would like you to work up on the following-
1) Chronic pancreatitis - A CT abdomen, fecal elastase will help. Usually associated with diabities
2) Crohn's disease - Usually associated with significant weight loss, a CT enteroclysis, serum CRP(quantitative), stool examination will help
3)Mesenteric adentitis or epiploacae appendicitis - visible on a CT, associated with pain andomen and fever
4) Vasculitis - may be associated with renal and skin lesions
Hence donot get depressed and go XXXXXXX a GI and discuss these possibilities and work towards a solution

I Appreciate this greatly. I also just found out I was pregnant yesterday which may be why the pain has gotten worse. Are these tests still safe to be done during pregnancy?

Hi and congratulations
In pregnancy CT and X rays are not safe
Some lower abdominal pain can occur in pregnancy but pain not allowing you to stand straight is unusual. Hence get yourself examined to rule out other problems as well as ectopic pregnancy.
