Am I At A Risk Of Developing Prostate Cancer With A PSA Level Of 2.7?

Question: I am a 64 year old man with a very enlarged prostate (120cc) volume. Had urinary problems hematuria and an episode of urinary retention. Biopsy in Oct 2017 negative. Have been on finasteride for a year. Latest PSA was 2.7 with a free percent PSA 30%. Wanted to know what the chance is that I have prostate cancer?

Hello,

Based on the facts that you have posted you seem to have Enlarged Prostate (122cc).

In view of your, PSA being normal and biopsy being negative there is no possibility of Cancer of the prostate.

Continue medications that you are taking and monitor the size of the prostate and PSA at regular interval of every 3-4 months.

I am concerned however that the PSA might really be elevated since finasteride can lower PSA by 50% as I understand. Would it be worth it to get the multiparametric MRI for the prostate.

Finasteride does not have action on the secretion of PSA by the prostate.

Hello,

Finasteride is an alpha reductase inhibitor and reduces the production of Dihydrotestosterone (DHT) which is the hormone responsible for the enlargement of the prostate and thus helps to reduce the size of the enlarged prostate. It does not have action on the secretion of PSA by the prostate .
