Are HIV I And II Tests Done 3 Months Post Exposure Conclusive?

Question: I had a protected sexual encounter wit no oral sex with a call-girl on April 12th and had a HIV I and II Test which found to be non-reactive by TRIDOT method on 06.07.15.Its well past 3 months and want to know how accurate it is?Are more tests necesssary?

Also had a USG of neck and Kindly find below the complete ultrasonography of neck:-

1.THYROID: Both lobes of the thyroid and isthmus are normal in size and shape.No focal solid or cystic lesion is noted.No Calcification is noted.

2.BILATERAL PAROTID:Are normal in echogenicity. No focal lesion is noted.No evidence of any ductal dilatation seen.No calcification or ductal calculus seen.The adjacent fat planes are maintained.

3.BILATERAL SUB MANDIBULAR SALIVARY GLANDS: Bilateral submandibular salivary glands are normal in size,shape and echogenicity.No calcification or ductal calculus noted.

Well defined hypoechoic nodular lesions are seen in both sides of neck at the level of angle of mandible.lesions of right side measures 8*12.7mm and6.8*10.8 mm.the lesions of the left measures 9.2*17mm.Evidence of necrosis is noted in the left sided lesion.

IMPRESSION: Bilateral level II Cervical lymphadenopathy

I have gone through your question. I can understand your concern.
You have negligible chance of HIV so don't worry about that. Your test is quite sensitive but for safe side you should go for testing after 6 month of exposure.
Go for 4th generation ELISA test or PCR.

Your thyroid, parotid and sub mandibular glands are normal.
You have enlarged lymph nodes. Your ultrasound report suggest evidence of necrosis. So you have high chance of tuberculous lymphadenitis. It can be tuberculosis in lymphnode alone or secondary to lung tuberculosis.

You should go for fine needle aspiration cytology(FNAC) of that enlarged lymphnode and x ray chest. It will give you exact diagnosis. Then you should take treatment according to diagnosis.

Duration of treatment of tuberculous lymphadenitis is generally 6 months.
It is completely curable.
Side effects are liver toxicities mainly. So you should go for base line SGPT (liver enzyme) level before starting treatment. Other major side effect is neuropathy. For that vitamin B 12 supplements are taken.

Symptoms of tuberculosis lymphadenitis are enlarged lymphnode, mild fever, weight loss etc. However you may not have all symptoms.

It is not contagious so don't worry about that.

I have gone through your question. I can understand your concern. It is more common in hiv. Because in hiv there is immunodeficiency. So it leads to many infection. Tuberculosis is one of the most common infection in HIV patients. But it occur in later stage of hiv when cd4 count is low.

You should not worry about HIV at this stage. Your test comes negative so just be relaxed.

Thanks for the information.
I had a visit with a medicine doctor and he gave antibiotics for a week and visit after 10 days,
3 days have already passed and I am getting a bit of pain near my neck region.Is it due to the antibiotics?
Is FNAC is safe and are there any tests to confirm this type of TB excluding FNAC?

Pain is due to some inflammation. Don't worry much about that.
Your doctor's give you a trial course of antibiotics. If it is reactive hyperplasia of lymphnode due to some other infection then it will cure. If it doesn't respond to treatment then there is high chance of tuberculosis.

FNAC is very safe procedure. It doesn't require any anesthetia. It will just take 2 min to do FNAC. So don't worry about that.

Other option is biopsy. But according to my opinion FNAC is best to confirm tuberculous lymphadenitis.

If lymph node TB occurs means it is extra pulmonary TB? I want to know if the same bacteria cause both the pulmonary tb and extra pulmonary ?
Is it safe to have sex with someone in my case?

If lymphnode tuberculosis occur it means it is extra pulmonary tuberculosis.

It is caused by same bacteria mycobacteria tuberculosis which causes pulmonary tuberculosis.


You can have sex. There is no harm. It is not sexualy transmitted.

If not treated it leads to spread of disease. It also leads to weigh loss, reduce immunity and its spread to other organ leads to organ dysfunction.

So just go for testing and if it confirm yhe diagnosis then take complete course of anti tuberculous treatment. It will cure the disease completely.
