Brief Answer:
Muscle pain / Counselling will help

Detailed Answer:
Hi friend
Welcome to Health Care Magic

There is nothing to suggest heart, as the cause of your pain. Pain from the heart is oppressing, in central chest (and left), worse on effort, relieved by rest, may radiate to shoulders and arms, associated with sweating and so on.
Norma; ECG and ECHO are reassuring
Your symptoms do not suggest Coronary Artery Disease (CAD). However, it is better to have a Treadmill Exercise Electrocardiogram (TMT). If it is normal, nothing further to do – done more to rule out rather than rule in..
An upper Gastro Intestinal Endoscopy (Gastroscopy) can rules out issues of food pipe and stomach.
This is most likely to be pain from muscles of the chest wall – and this could be from spasms from posture (say during sleep) or stress, and so on.
Psychological assessment and assistance will help a lot. No need for hesitation to see a Psychiatrist – everyone seeing a heart specialist need not have a heart disease. They can assess and assist the role of depression or anxiety... and suggest appropriate medication or counselling

Good luck
God bless you

Brief Answer:
No test needed for heart

Detailed Answer:
Hi

Normal stress test excludes heart, as the cause of the pain
NO FURTHER TEST IS REQUIRED FOR THE HERT
Cervical spine problem pressing on a nerve root can cause radiation .. MRI of the neck spine and neurological consultation will clinch the diagnosis
Great to have stopped tobacco and alcohol. After 5 years, there is no bad effects and it is like normal persons. Keep it up

Follow up with your physician / Endoscopy and Psychiatric assessment
Prognosis is generally excellent in such situations
