Are Fever And Cold In A Child Curable With Basic Medication?

Question: My son, who is 4yrs 3months old is having cold for about 7days and got fever from Sunday afternoon. I gave P250 and Noset for fever and cold on Sunday noon. Then he was fine in the evening. Next day when he got up his temperature was around 102.5 degree. Then we took him to hospital. They Dr gave fevastin 0.5 injection ,But during night his temperature raises to 103 degree.

Evening again we consulted doctor and they suggested to continue the same medicine. Tuesday day time, temperature was normal but again Tuesday night temperature rose to 102. Wednesday again we took him to hospital and took blood and urine sample for suggested reports. Have attached the reports and current medications for the same.

Hi,

But what you say I feel that your kid is having viral illness.
I will explain you how a viral illness behaves so that you can be more confident.

Fever of few days without any localizing signs could as well a viral illness. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day. If the kid is active and playing around when there is no fever, it is probably viral illness and it doesn't require antibiotics at all. Once viral fever comes it will there for 4-7 days. So do not worry about duration if the kid is active.

Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

I do not recommend giving paracetamol injections for fever because - whether you give it by oral route or suppository or as an injection, the action will last only 4-6 hours and we cannot keep giving a painful injection for the kid every 4-6 hours once.

Can you upload prescriptions and investigations.

I have attached the blood work and also the medicines we are giving to him. Apart from this he is having RD60 half tablet morning and night. He vommitted twice today and then he is telling that he has stomach and leg pain. In the blood work, typhoid is positive. Can you plz confirm or we need to wait for atleast 7 days to confirm it is typhoid?, bcoz today is the 4th day for his fever.

Hi,

I feel the Widal test which you have performed is slide agglutination test which is not appropriate test to detect typhoid.

Widal test should be performed in tube method and the too on 8th day of fever with there is consistent fever for 7 days.

You are very much right in waiting for doing a test for Typhoid after 7 days and that too we need to go with the gold standard test of blood culture rather than Widal.

But you have already used an antibiotic which is unfortunate.

I still feel that this is a viral illness and you can wait by plotting a temperature chart every 4 hours once for another two days before going with any of the blood tests.

Hello Dr,

What about the medicines am giving, is it fine? Do u want me to make any changes to that? Also would like to know, y is it unfortunate for antibiotics? Is there any protocol for giving antibiotics and taking any blood test? Plz explain.

Few more doubts, 1) is he having urinary infection? Is that causing fever bcoz the report shows pus cells . 2) how long does an infection in the blood takes to spread all over the body and affect the organs? 3) if the fever persists, dr asked me to give the Amkee 125 injection, is it ok to give? Is it an antibiotic? 4) what is the best way to handle fever and the procedure we should follow, bcoz as a mom we always get tensed and run to doctor and happen to fed him around 5 to 6 syrup?

Hi,

1. There is no need to give cefpodoxime antibiotic.
2. As I had suggested give only Paracetamol for fever and nothing else.
3. For cough and cold there is no need to use any combination medicine like this. You can use only promethazine available as phenergan as a single medicine.
4. I told antibiotic usages unfortunate because I feel that it is Viral which does not warrant antibiotic therapy. Misuse of antibiotic leads on to antibiotic resistance.
5. He is not having urinary tract infection because this urine routine report is normal.
6. The duration taken by the infection to spread all over the body through blood depends upon the virulence of the organism like virus or bacteria or fungus and how virulent is the organism.
7. Amikacin is an antibiotic and I don't think just because fever is persisting we need to give another antibiotic without proper investigation.
8. As I have explain you already how a viral illness behaves, just keep those points in mind and give only paracetamol if the fever is above hundred degrees Fahrenheit and if the baby is active when the fever touches the baseline, you can wait for 5 days without giving anything else except Paracetamol.

A 4 year old child weighing 12kg is underweight, I agree with you.

But one basic dictum is that an active child is a well child.

So if the kid is active and developmentally normal, I don't think you need to worry about his weight please.
