Are Lesions In The Lower Left Part Of The Lungs According To A CT Scan Dangerous In A Smoker?

Question : Hello Doctor,
I want to ask about my dad.
He is 69 years Old. He was a Smoker (stopped from past 3-4 months).
1 month back in a CT Scan report we came to know he is having a 'Lesion' in his lungs.
Size - 2.8 x 1.9 x 1.2 cm
Location - Left, lower part of the lung.

We under went for a Biopsy of it. (Report attached)
The doctor said No need to worry about it.

I'm here for a second opinion. Is this Lesion dangers? Please advice.

Get done repeat biopsy.

Detailed Answer:

Hi,

Negative biopsy report does not 100% rule out malignancy.
If biopsy material is not from core of lesion, this can gives false negative results. This is very common when lesion is less than 3 cm in size.

In my opinion, you have 2 options.
1. Get done repeat biopsy now. Also get done bronchoscopy and BAL (bronchoalveolar lavage) for cytology examination.
2. Wait and watch for six - eight months. Get done repeat CT thorax. Compare the size of nodule. If worsening then get done biopsy.

Please let me know
1. What are his symptoms?
2. Do you have any previous chest x ray?

He does not have any symptoms like fever, cough etc ..
He had weight loss, doctor said it is because of uncontrolled sugar (450-500). Last month before treating to sugar he weight was 56kgs, now he is 59kgs.

Can you refer me to any doctor in Bangalore?

Hi,

Since he is having uncontrolled diabetes, we should also need to rule out tuberculosis. Smoking history is in favor of malignancy while diabetic history is in favor of infective etiology more.

They had done ECHO. Report was Normal.
Bronchoscopy - He did not talk about it.

Also, my dad had Altered Bowel habits. So, OGD, Colonoscopy were done - reports were normal.

We went for follow up check up on 2-sep-2018, he did not talk about it. He just changed Diabetics tablets and asked to check for every 3 days.

Hi,

In my opinion, you should definitely consult pulmonologist first. If I am his treating pulmonologist then I would go for:
1. Repeat biopsy to rule out or confirm malignancy and tuberculosis.
2. If repeat biopsy is not conclusive then bronchoscopy and BAL should be done.
3. If this is also inconclusive then better to start anti tubercular drugs and get done repeat CT thorax after 6 months.

Is he having any family history of malignancy?
Is he have any work place exposure to TB patient?

Hi,

You should first consult pulmonologist. Bronchoscopy and BAL are next step. Repeat biopsy can also be think of first. Prophylactic anti tubercular drugs should be started if both (bronchoscopy and CT guided biopsy) are negative for malignancy. Review CT thorax after 6 months.

Can you send me picture of CT thorax film? This gives better idea.
