Brief Answer:
Cause of concern, needs evaluation and investigations

Detailed Answer:
Thanks for your feedback.
Such a small aneurysm is usually not known to cause such a pain that is radiating to the back and left shoulder.
There is something that is irritating the diaphragm that can cause such symptoms.
The probable causes are"
Pancreatitis.
Gastric ulcers on the posterior wall.
Aneurysm if dissecting.
Any collection in spleen or under diaphragm.

I think this is a cause of concern and needs proper clinical evaluation, examination and investigations to get the proper cause to get a proper management if needed as needed.
Consult a Doctor as possible and get the above things done asap to know the cause.

I hope this answer helps you.
Take care.

With Regards.
