Question: hi,

My mother is 67 Years old and recently had an angioplasty around 2 months back, everything was fine till now..she had some spots like blood clots on her hands but as doctors said it was due to heavy medication and now its fully recovered when they reduced the no.of medicines..but since last 3 days she is feeling a pinching sensation while sleeping and turning around..Is it normal?? Pls help us out..I am really worried on this.. I saw somewhere that they might have to re-operate again and I really can not see my mom going through the hell lot of pain..

Brief Answer:
several possibilities can be there..

Detailed Answer:
Hello XXXXX, Welcome to HCM,
Sorry for the delay,

I've gone through your query in details and understand your concern for your mother,

Now when you said "they reduced the no.of medicines" can you please clarify which medicines are going on and which is stopped (it's extremely important as they can be a consequence of that)..

I don't want to scare you but you've read it right, because your mother is in that time phase where chances of re-stenosis/ new clot formation is quite high (the first 6 months which is standard risk factor for almost every angioplasty patients)..

It's extremely unlikely that the stent itself started irritating the vessel so need not to worry about that..

There are less severe concerns like neuro-muscular spasm, respiratory tract infection etc. where the same can be experienced but the most relevant and most important concern has to be addressed at first..

So, there's a need to inform her cardiologist about this without any further delay, to run a few necessary investigations (Chest X-ray, repeat 2D-Echocardiogram along with color doppler in and around the stent, coagulation profile repeat) for the prompt check-up in the first place and proceed accordingly..

Please don't worry, but do the needful as it's essential to rule this out in the first place in view of the precious heart and recent angioplasty..

Let me know if there's any further queries, i'll be glad to clarify..

Take Care
Kind Regards

Hi

Thank you very much for the reply

I am attaching the ECG report she has taken up today. Pls tell me if its normal any concern??

Regards
XXXXXXX

also regarding the medicines:

1. Drego D
2. Novostat
3. Starpress
4. Ecosprin 75 mg
5. Axor

They have given all these for 1 week (all 1 per day, axor twice per day)

Then they have reduced it to the following after the one week follow up check up:
1. Axor (twice a day)
2. Ecosprin 75 mg (once a day)
3. Starpress (once a day)

(they initially prescribed for 3 months, now after the 1 month check up they are reducing it to only ecosprin)

And after 1 month follow up, now they have reduced it to:

1. Ecosprin gold (10 mg) (twice a day) for next 6 months

But my mom is still following the previous medications and after the 3 months check up only she will use these last prescribed medicine (only ecosprin).

Pls let me know is it normal or safe

Regards

Brief Answer:
follow-up consultation..

Detailed Answer:
Hello XXXX, Welcome back,
Thanks for following up with the details,

First of all, to avoid any unwarranted anxiety please don't discuss these things in front of your mother..

Secondly, you should have faith in your cardiologist, she seems to be in good hands only..don't worry..

Her medications seem fine (is it AXOR or AZOR ? Dosage ? )..
Switching over to 'ECOSPRIN GOLD' is a wise approach as it improves drug compliance (it's a combination drug, now she has to take just one instead of three), so please follow them as advised and kindly don't alter the schedules on your own..

Also, please continue DREGO-D once daily in empty stomach (15-20 min prior to breakfast) for another one month..

No attachments/ECG report found here, so can't really comment on that..

However if you go back to my previous answer, you'll find that I never mentioned of ECG, as it's not really relevant in this scenario (ECG won't pick up any thing significant unless patient is grossly symptomatic, i.e. recurrence of chest pain, breathing difficulty, sweating and those alarming symptoms, which you yourself must be well aware of by now)..

Please inform her cardiologist about this without any further delay, to run a few necessary investigations (Chest X-ray, repeat 2D-Echocardiogram along with color doppler in and around the stent, coagulation profile repeat) for the prompt check-up in the first place and proceed accordingly..

Please don't worry, but do the needful as it's essential to rule this out in the first place in view of the precious heart and recent angioplasty..

Let me know if there's any further queries..

Hi Sir,

Thanks for the details.

We consulted with the cardiologist after her recent ECG today, he said its normal.
But he has given medication of:

1. Pentocid DSR
2. Dolo 650

Both to be taken twice daily for 3 days.

Is this relevant to the issue she has?? I am really confused. That's why seeking your opinion on this.

Brief Answer:
don't worry, since already consulted with her cardiologist..

Detailed Answer:
Hello XXXX, Welcome back,

No need to worry, since already consulted with her cardiologist..

Since PANTOCID-DSR is advised already then no need to take DREGO-D, yes continue the DOLO also for next 3 days, but if symptoms persist beyond 3 days, do get her checked up thoroughly and discuss with the cardiologist regarding those necessary investigations as I've already explained in details..

Just Keep her under supervision, let her take all the cardiac medication exactly as advised by the cardiologist and don't stress yourself too much, it should be fine within a matter of time only..

Best Wishes..

Take Care
Kind Regards
