Question: Have been feeding both lactogen and breast milk for baby does it have side effects? How much formula to give as my milk is not sufficient for her? Wil they be constipated as she does not have regular everyday bowel movement? She is 4 weeks old till date

Hi,
Thanks for writing to us.

At 4 weeks usually there will be enough breast milk output for most of the mothers. However if your milk output is less you can add lactogen. No problem. No side effects.

Mild constipation can occur, but usually improves in a few days.

Always breastfeed each time for 20 minutes, after that if baby is still hungry you can give 30 to 50 ml extra. Stop once baby refuses the feed.
Ensure she passes urine atleast six to eight times a day.

Hai doctor ur suggestions were a relieving for me as it's my first child I just a one more question my daughter seems to be colicky she cries excessively in the evening with clenched fists and a flushed face she doesn't feed properly at those times and she is very gassy passes lot of gas doc we tried colicaid it works out well but what's the dosage and how to manage colicky kid everyone here are getting frustrated with her cries and in turn reduces my milk supply due to stress are there any solutions how long does it take to reduce and does lactogen causes colic doc?

Colicaid 0.3 ml thrice a day before feed for a week is what I recommend to my patients.
Colic pain may last upto 3 to 4 months.
Proper burping after each feed should help.
Don't worry it will settle down soon.

I feed my daughter my milk but since doesn't latch on properly sometimes I have to express and feed her i am able to express around 15 ml each time and give 25 ml of lactogen with half a scoop I feed around every hourly or second hourly will this be sufficient? Usually is there a measured volume of breast milk given for an 4 week old infant? Should I decrease or increase the formula?

If baby is gaining weight and passing urine well as mentioned above, it means that the feed you are giving is adequate. I think that what you are giving should be OK because you express sometimes and feed formula one to two hour once.

If not increase the formula.

4 week old infant.can.drink. 120 ml usually (breast and formula put together during each feed) Every 3 hours

My daughter 4 weeks old is awake from 11 to 3 in the night and she wants to feed every half a hour and goes to urine more and cries again for feed . I have hypothyroidism from childhood and is on eltroxin 100 micrograms I feel my milk is less and give her lactogen 25ml with almost a scoop is it correct wht is the reason for crying at night and do infant feel lot hungry at night? It is very normal for infants to be hungrier at night.
They tend to be more active and their demand goes up in the night
This day night rhythm changes slowly and by 6 to 12 months they start resting more at night.
Don't worry.
You mentioned that she goes urine often which indicates that you are giving her adequate milk including formula.
Try increasing formula to 60ml /2 scoops.
Always add one scoop to 30 ml as recommended by the manufacturer.

Usually on eltroxin if hypothyroidism is well controlled it shouldn't affect milk production.
You can test your thyroid profile to make sure that it it is well
controlled.
To increase milk output
1. Drink plenty of milk and liquids.
2. Try perinorm tablet thrice daily for 5 days only.
3. Feed in calm and relaxed environment.
4. Latching - Baby's mouth should engulf nipple and areola together atleast half the areola should be inside baby's mouth. This ensures proper sucking. Sucking only at nipple will not release much milk.

At this age 120 ml each feed for lactogen maximum 6 times, which would come to 720ml per day.
Mild engorgement is common, it is fine to express and feed.
Expression of milk will not cause engorgement.
Mild pain during let down reflex is common.
It should settle as milk starts flowing.
However if pain persists you need to get examined.
Engorged breast will be warm tender sometimes with fever.

Colicaid has fennel oil and dill oil which have anti gas properties.
Both help in relieving colic pain.
Colimex will help more if the pain is severe.
For mild pain and gas issues colicaid is enough.
I usually give colicaid drops 0.3 ml four times a day before feed for my patients.

Bcg vaccine can cause fever upto few days after vaccination not at 3 weeks.
Generally most Vaccines can cause mild fever for one or two days after vaccination.
Some live Vaccines like measles, chickenpox, mmr can cause slightly more fever.
All you need to do is give paracetamol drops. I usually prescribe around 0.5 ml thrice daily at this age.

Hai doc ur r ply was very helpful my daughter passes motion regularly these days at least sometimes alternate days but her stool is some what greenish to yellowish with some particles that looks like curdled ones is it related to formula milk lactogen and feeds on lactogen very often every hour at night as she goes hungry she is five week old is her tummy upset or is she allergic to something or intolerant to lactogen? I don't know what to do she takes 50 to 60 ml formula milk

If your baby is crying before passing urine it could indicate urinary tract infection. It could also be due to heat and less intake.
I suggest that you feed frequently and get urine tested
I usually prescribe urine routine and culture test after cleaning the area well with mild soap and water.
If the reports indicate infection she may require antibiotics.
Otherwise it will settle with more frequent feed.
